Your Role
Reporting to the Director of Health and Wellness, the Mental Health and Addictions Case Worker will be responsible for integrating the four elements of the Values and Virtues of Life (mental, emotional, spiritual, and physical) into the on-land family and children supports provided by Family Council.

Duties and Responsibilities
- Maintaining regular (bi-weekly at the least) contact with all clients and soon to be clients; contact will fluctuate as client needs or circumstances change;
- Have a significant knowledge of mental illness under the DSM;
- Document all interactions with clients on their individual client files, maintaining both physical and electronic files;
- Ensure that clients are notified promptly of community events and opportunities, C/TFN communications intended for clients/citizens/community members, Federal or Territorial changes to services, especially as it relates NIHB or senior services, Health and Wellness specific initiatives, or other such communications where the client can benefit spiritually, emotionally, physically, or mentally;
- Provide addiction and transitional support to clients in withdrawal management, admission/discharge from medical services, treatment services of other institutions;
- Encourage harm reduction, abstinence or other avenues of addiction support to clients;
- Develop implement and facilitate workshops and programs for clients/citizens/community members that are aligned with the values and virtues of C/TFN;
- Advocate for clients within all domains of their lives through support for self-determination;
- Work with family members of clients to coordinate areas that the client is having difficulty with, ensuring confidentiality and privacy are open as possible, and closed as necessary;
- Administrative Duties, not limited to statistical reporting each month and file maintenance
- Create a policy in collaboration with the Director and Manager;
- Complete all other duties as directed by the Health and Wellness manager and director, which will include, but is not limited to seeking out and applying for funding opportunities, support in stand alone or departmental projects, etc.

- Good understanding of determinants of health and well-being for vulnerable people;
- Substantial knowledge of mental illness as defined by the DSM;
- Significant knowledge as it relates to addiction case management;
- Significant knowledge of C/TFN Family Act, Book 1 & 2;
- Must be able to incorporate the Values and Virtues, and traditional knowledge of Carcross/Tagish into all events, programs, curriculum, and interactions;
- Knowledge of the communities of Carcross, Tagish, Squanga, and Whitehorse, and C/TFN citizens,
- Ability to work effectively as an integral part of a team and foster a team approach to service delivery;
- Oral and written communication including presentations, briefings and reporting;
- Strong organizational and time management skills;
- Ability to work with minimum supervision;
- Ability to develop, coordinate, or facilitate workshops.
