JOB SUMMARY
Reporting to the Senior Manager of Health and Wellness, the Outreach Worker provides outreach support and support services to C/TFN Citizens in difficult or trying situations involving health issues, child and family matters, justice issues, addictions and/or other social issues.
DUTIES AND RESPONSIBILITIES

  • Establishing and maintaining positive connections with clients
  • Helping clients to obtain and interpret information regarding a variety of health and social issues
  • Making referrals to appropriate resources, programs and services including assisting with application and preparation for addiction treatment
  • Providing crisis support and intervention as needed;
  • Providing advocacy and support as needed for clients dealing with other C/TFN departments, government agencies, RCMP and other applicable agencies
  • Providing support for pre-treatment and after-care as needed
  • Attending and fully participating in staff meetings
  • Documenting client contact and pertinent information in case files and maintaining statistical records as required
  • Sharing information and collaborating with team members regarding client work
  • Participating in case management and consultation meetings with outside agencies in accordance with our confidentiality and release of information policies
  • Assisting with departmental work-plans and budgets
  • Managing and maintaining comprehensive client files and records all interactions in case management system (such as ShareVision), that are compliant with all current legislation and policy
  • Positively and professionally represents the Health and Wellness Department, and C/TFN with colleagues, citizens and community. Establishes and maintains effective working relationships
  • Demonstrates a commitment to personal growth and development to remain current and stay abreast of developments and opportunities and to exchange information
  • Assists with reporting obligations related to program funding
  • Participating in priority planning meetings.

ESSENTIAL QUALIFICATIONS

  • Post-secondary coursework in Social Programs, Counseling, Mental Health, Psychology, Social Sciences, Sociology or equivalent
  • Knowledge of Fetal Alcohol Spectrum Disorder and other mental disorders and ability to help educate others on the topic

DESIRED KNOWLEDGE, SKILLS & SUITABILITY

  • Experience in assessing and dealing with high risk situations
  • Knowledge of C/TFN Family Act, Book 1 & 2
  • Good knowledge of the communities of Carcross, Tagish, Squanga and Whitehorse
  • Ability to work effectively as an integral part of a team and foster a team approach to service delivery
  • Ability to establish good working relationships with C/TFN staff, citizens, Executive Council and Management Board
  • Well-developed oral and written communication including presentations, briefings and reporting
  • Strong organizational and time management skills
  • Ability to motivate and provide leadership, guidance and advise to participants and others
  • Ability to work with minimum supervision
  • Ability to coordinate or facilitate mini-workshops
