JOB SUMMARY
Reporting to the Senior Manager of Health and Wellness, the Aftercare Coordinator works collaboratively as part of the Health & Wellness team to support Citizens and community members on their recovery journeys. The Aftercare Coordinator is responsible for working closely with Citizens to create individualized aftercare plans and providing person-centered case management support to Citizens on all stages of their wellness path. The Aftercare Coordinator will be involved in developing community-based programming and initiatives, as well as linking people to existing supports.
DUTIES AND RESPONSIBILITIES

  • Work closely with individuals along the continuum of substance use to determine what wellness looks like to them and what their aftercare goals are, and then collaboratively develop holistic aftercare support plans
  • Assist Citizens in identifying, creating, and strengthening their individualized support networks
  • Advocate on behalf of clients and provide ongoing liaison and case management support
  • Provide referrals and advocacy services to individuals experiencing substance use related challenges related to relapse prevention, mental health, and concurrent disorders
  • Provide one-on-one support, including lay counselling
  • Plan and facilitate structured group activities and programming, with a focus on land-based and cultural activities
  • Facilitate the involvement of Elders and Community Leaders who have undergone their own wellness journey into the program as support
  • Develop, provide or participate in community education activities, such as awareness campaigns, workshops, presentations and events that address substance use
  • Maintain confidential intake forms, as well as manage and maintain comprehensive client files. Record all case notes and interactions into a case management system (such as ShareVision) in a way that is complaint with all current legislation and policy
  • Support with reporting as required to funders, Executive Council, Management Board, Elders Council, and others are required
  • Positively and professional represent the Health & Wellness Department and C/TFN with colleagues, Citizens, and the community
  • Participate in all events as requested by supervisor to fulfill the mandate of the department (such as health fairs, planning meetings special projects, etc.)
  • Demonstrate a commitment to personal growth and development to remain current and stay abreast of developments and opportunities
  • Maintain strict confidentiality at all times
  • Other related duties as required

ESSENTIAL QUALIFICATIONS

  • Preferred degree in Addictions Counselling, Social Work, or in a related human services field, with specialized training or experience in the area of alcohol and/or substance use issues (or equivalent work experience in the areas of crisis intervention, trauma counselling, or psychology)
  • Completion of Peacemaking Circle Training is encouraged and will be considered an asset

DESIRED KNOWLEDGE, SKILLS & SUITABILITY

  • Familiar with harm reduction principles and practices,
  • Experience working closely with First Nation communities,
  • Knowledge of Yukon First Nations history with Residential Schools, loss of culture and community healing, child welfare, impacts of intergenerational trauma and community outreach
  • Knowledge of C/TFN culture, history, demographics, goals and aspirations as well as a good understanding of the communities of Carcross, Tagish, Squanga and Whitehorse
  • Ability to work respectfully with others in a cross-cultural environment
  • Ability to work effectively as an integral part of a team
  • Excellent organizational and time management skills
  • Excellent case management, time management, and stress management skills
  • Demonstrated ability to engage with people in a way that build trust and rapport
  • Effective crisis intervention skills, including the ability to offer lay counselling support
  • Strong decision making and conflict resolution skills
  • Ability to foster a team approach to service delivery
  • Experience coordinating, planning and facilitating one-on-one and group support sessions
  • Ability to review and assist in evaluating policies related to the program
  • Excellent interpersonal communication skills including oral, written, and presentation
